Creatine occupies an unusual position in the supplement world. It has more human research behind it than almost any other supplement — thousands of studies, decades of safety data, consistent findings across different populations. And yet it’s still surrounded by myths: that it’s primarily for bodybuilders, that it causes kidney damage, that expensive forms are better, that loading is necessary. None of these are well-supported. Let’s go through what’s actually established.
What creatine does in the body
Creatine is stored in muscle (and the brain) as phosphocreatine, where it serves as a rapid-access energy reserve. When your cells need a quick burst of energy — during high-intensity exercise, or demanding cognitive tasks — phosphocreatine donates a phosphate group to regenerate ATP, the cell’s energy currency. Supplemental creatine saturates these stores beyond what diet alone achieves, extending the duration you can sustain high-intensity effort before fatigue.
Your body produces creatine naturally, and you get some from meat and fish in your diet. Vegetarians and vegans have lower baseline creatine stores (because they don’t eat the dietary sources), which makes supplementation particularly beneficial for them — and makes creatine one of the few supplements with a stronger rationale for plant-based eaters than omnivores.
The evidence beyond athletics
The athletic evidence for creatine is among the strongest in all of sports nutrition. But increasingly, the research is widening. Studies in older adults consistently show benefits for muscle mass and strength retention — particularly relevant given that sarcopenia (age-related muscle loss) is one of the more consequential health shifts of ageing. Maintaining muscle into older age is associated with better metabolic health, reduced fall risk, and greater independence.
Cognitive research is earlier and more limited, but intriguing. The same phosphocreatine system that supports muscle energy also operates in the brain. A number of studies have found measurable improvements in cognitive performance — particularly under sleep deprivation or mental fatigue — with creatine supplementation. This is not a settled field; the effect sizes are modest and the research ongoing. But creatine is no longer just an athletic supplement, and the evolving evidence for cognitive and ageing applications is worth watching.
The kidney myth
Creatine supplementation raises creatinine levels in blood tests — creatinine being a breakdown product of creatine metabolism. Because creatinine is one of the markers used to assess kidney function, this has caused concern that creatine damages kidneys. The concern is not well-founded in healthy individuals. Elevated creatinine from creatine supplementation reflects higher creatine turnover, not kidney damage. Long-term studies on healthy people show no adverse renal effects at standard doses. People with pre-existing kidney conditions should discuss creatine with their doctor; for healthy individuals, the kidney concern is not evidence-based.

The form question: monohydrate vs everything else
Creatine monohydrate is the form used in the vast majority of research. It’s also the cheapest form. This is the genuinely unusual situation where the evidence points directly at the economical option. There is no well-powered research showing that creatine HCl, buffered creatine, creatine ethyl ester, or any other proprietary form outperforms monohydrate. These forms exist to create product differentiation at premium prices. Monohydrate, particularly micronised monohydrate (for better solubility), is the rational choice.
Loading — necessary or optional?
A loading phase — taking 20g per day in divided doses for 5–7 days before dropping to a maintenance dose of 3–5g — saturates creatine stores faster. If you need rapid results (say, before a competition), loading achieves full saturation in about a week. If you take 3–5g daily without loading, you reach the same saturation in about 3–4 weeks. For most people supplementing for general health and performance, there’s no practical reason to load. Skip it unless you have a specific short-term reason to accelerate the timeline.
People who perform high-intensity exercise (sprinting, weightlifting, team sports). Vegetarians and vegans with lower dietary creatine intake. Older adults looking to support muscle mass and function. People doing cognitively demanding work, particularly under fatigue. Almost anyone who trains and wants one of the few supplements that consistently shows performance and health benefits in the evidence base.
Plain creatine monohydrate. Micronised if available, for better mixing. No loading required. 3–5g daily, any time of day that’s convenient — the timing sensitivity is low compared to, say, iron or fat-soluble vitamins. That’s it. Resist paying extra for any form that isn’t monohydrate — the research doesn’t support the premium.
