Iron deficiency is the most common nutritional deficiency globally, and iron supplementation is among the most frequently recommended medical interventions for women, vegetarians, and those with certain health conditions. It’s a supplement where the stakes are real — deficiency causes genuine harm, and correcting it matters. Which makes it particularly frustrating that most people are given a dose recommendation without adequate guidance on form, timing, and the interactions that determine how much of that dose actually reaches their cells.

After 20 years in supplement quality, here is the practical science behind iron supplementation that most product labels leave out.

Don’t supplement iron without testing first

This deserves its own paragraph before anything else. Iron has a narrow therapeutic window — too little causes deficiency, too much causes oxidative stress and organ damage over time. Unlike most vitamins and minerals where supplementation in healthy people carries low risk, iron supplementation without confirmed deficiency is not a harmless act. The appropriate test is serum ferritin (iron stores), not just haemoglobin, because haemoglobin can appear normal while ferritin is depleted — what’s known as iron deficiency without anaemia, which still causes fatigue and cognitive symptoms.

If a blood test shows you need iron, supplementation is entirely reasonable and effective. If you’re supplementing speculatively because you feel tired, get tested first.

Form — the biggest variable in iron supplementation

Ferrous sulfate is the standard form — cheap, well-absorbed at about 20% in iron-depleted individuals, and available everywhere. It’s also the form most associated with the gastrointestinal side effects that cause people to stop taking it: nausea, constipation, dark stools, stomach pain. These are real enough that adherence to ferrous sulfate is a genuine clinical problem.

Ferrous bisglycinate (iron bisglycinate or iron glycinate) is a chelated form — iron bound to the amino acid glycine. It’s absorbed at a comparable or slightly higher rate, typically with significantly fewer gastrointestinal side effects. The research comparing the two is consistent on this point: bisglycinate produces similar iron repletion with better tolerability. It costs more and appears less often in standard pharmacy products, but for people who can’t tolerate ferrous sulfate, it’s a meaningful alternative worth seeking out.

Ferric forms (ferric sulfate, ferric pyrophosphate) are less well-absorbed than ferrous forms in most cases, though some newer ferric products use specific delivery mechanisms to improve this. They’re worth a lower priority unless specifically recommended by a healthcare professional for a particular reason.

Timing: the interactions that determine what you absorb

Iron absorption is significantly affected by what you take it with. Understanding these interactions can double or halve the effective dose you actually receive:

Take with vitamin C. Vitamin C reduces ferric iron to ferrous iron in the gut, dramatically improving absorption. Supplementing 100–200mg of vitamin C with your iron dose is one of the most consistently supported ways to enhance iron uptake. Orange juice works for the same reason.

Avoid calcium and dairy within 2 hours. Calcium directly inhibits iron absorption. A glass of milk taken with an iron supplement substantially reduces how much iron you absorb. This includes calcium supplements — separate them by at least 2 hours from your iron dose.

Avoid tea, coffee, and phytate-rich foods at the same time. Tannins in tea and coffee bind iron and inhibit its absorption. Phytates in wholegrains and legumes do the same. Take iron between meals or away from these foods where possible.

Take on an empty stomach if tolerated, or with a small, compatible meal if not. Empty-stomach iron maximises absorption; food reduces it by 40–60%. For many people, the choice is between less-efficient absorption with food or no iron at all because of side effects on an empty stomach. Tolerability that enables consistency beats optimal timing with poor adherence.

Every-other-day dosing — a practical insight from recent research

A practical finding from recent research: iron supplementation every other day may produce similar or better iron repletion over time compared to daily dosing. The proposed mechanism is that daily iron supplementation triggers a hormonal response that temporarily reduces iron absorption for up to 24 hours, while alternate-day dosing avoids this. This doesn’t mean everyone should switch to alternate-day dosing — but if you’re supplementing and progress feels slow, it’s worth discussing the frequency, not just the dose, with your doctor.

The practical summary

Test before supplementing. If you need iron, choose ferrous bisglycinate if ferrous sulfate causes side effects. Always take vitamin C alongside it. Separate your dose from tea, coffee, calcium, and dairy by at least 2 hours. Take between meals if tolerated. And check your ferritin again after 3 months to confirm the supplementation is working — not just the haemoglobin, the ferritin stores.
