Probiotics: How to Read a Label That’s Almost Designed to Confuse You

Probiotics are one of the most confusing supplement categories on the shelf — and I don’t think that’s entirely an accident. Between the strain names, the CFU counts, the storage requirements, and the broad health claims, the probiotic aisle asks consumers to evaluate a product on terms most have no way to assess. After 20 years in this industry, here’s how I cut through it.

The short version: probiotics can genuinely help in specific situations, the label is unusually important and unusually hard to read, and a large amount of what’s sold is either underdosed, mismatched to its claims, or unlikely to survive the journey to where it needs to work.

CFU — the number everyone quotes and few understand

CFU stands for colony-forming units — the count of live, viable microorganisms in a dose. It’s the number plastered on every probiotic label: “10 billion CFU,” “50 billion CFU.” Bigger numbers are marketed as better, and consumers have learned to shop by this figure.

The reality is more nuanced. CFU count matters, but more is not automatically better, and the number on the label is subject to several catches. First, the CFU is often measured at the time of manufacture — not at the end of shelf life or at the point you actually take it. Live organisms die over time. A product claiming “50 billion CFU” at manufacture might deliver a fraction of that by the time you swallow it months later.

The label detail that matters: does it guarantee CFU “through end of shelf life” or “at time of manufacture”? The former is a meaningfully stronger commitment. If a product only guarantees the count at manufacture, the number you’re paying for is the best-case starting point, not what you’ll receive.

Strains matter more than the total count

This is the single most important and least understood point about probiotics. “Probiotic” is not one thing — it’s a vast range of bacterial (and some yeast) strains, each with different effects. The benefit of a probiotic is strain-specific. A strain studied for antibiotic-associated diarrhoea is not interchangeable with one studied for IBS symptoms, which is different again from one studied for general digestive comfort.

A proper probiotic label specifies the exact strains, usually as genus, species, and strain designation — for example, Lactobacillus rhamnosus GG, or Saccharomyces boulardii. That third part (the strain code) matters because the research is conducted on specific strains, not on the species in general.

The label red flag

If a product lists only broad genus names — “contains Lactobacillus and Bifidobacterium” — without specifying species and strains, you cannot match it to any evidence. It’s the probiotic equivalent of a proprietary blend: impressive-sounding, unverifiable, and impossible to connect to a specific benefit. Quality probiotics name their exact strains because the strains are the entire point.

The survival problem

For a probiotic to do anything, the live organisms have to survive stomach acid and reach the intestine viable. This is a genuine challenge, and it’s where many products quietly fall short. Some strains are naturally more acid-resistant than others. Some products use delayed-release or acid-resistant capsules to protect the organisms through the stomach. Many don’t, relying instead on a high starting CFU to ensure enough survive.

The label clues: mentions of acid-resistant or enteric-coated capsules, or strains specifically selected for acid and bile tolerance, suggest a product designed with survival in mind. Their absence doesn’t mean the product fails — but it’s a quality differentiator worth noticing.

Storage — the requirement people ignore

Many probiotic strains are sensitive to heat and moisture. Some require refrigeration; others are shelf-stable through specific manufacturing techniques (such as freeze-drying). A probiotic that requires refrigeration but has been sitting on a warm shop shelf — or in your bag for a day — may have lost much of its viability before you ever take it.

Check whether your product requires refrigeration, and if so, whether it was actually refrigerated at the point of purchase. A refrigerated-strain probiotic sold from a room-temperature shelf is a warning sign about either the product or its handling.

When probiotics genuinely help — and when they’re optional

The evidence for probiotics is strongest in specific, targeted situations: alongside or after a course of antibiotics (to reduce antibiotic-associated diarrhoea), for certain digestive conditions, and for some specific strains with conditions they’ve been studied for. In these cases, matching the right strain to the situation is what makes the difference.

For a healthy person with no digestive issues, a daily probiotic as general “gut wellness” has much weaker evidence — and a diverse, fibre-rich diet that feeds your existing gut bacteria (the prebiotic approach) is generally a more reliable foundation than a supplement. Fermented foods like yoghurt, kefir, and sauerkraut also provide beneficial organisms in a food matrix. Supplements earn their place for specific reasons, not as a default.

Read past the CFU number. The strains are what matter — they should be named specifically and matched to a reason you’re taking them. Check that viability is guaranteed through shelf life, not just at manufacture, and that storage has been handled properly. And be honest about whether you need one: for specific situations like post-antibiotic recovery, a well-chosen probiotic helps; for general wellness in a healthy person, a fibre-rich diet and fermented foods are a more reliable foundation than most supplements on the shelf.
